Where a physician determines, in their independent medical judgment, that it is clinically appropriate, the physician may provide medical documentation that you can submit to an airline, hotel, travel insurer, or other third party.

The Service is not a substitute for an in-person relationship with your primary care provider or for emergency or ongoing medical care.
All documentation is provided only after a licensed physician conducts a good-faith medical evaluation appropriate to the circumstances. Documentation reflects the physician's honest, independent clinical assessment. We do not, under any circumstances, create false, misleading, backdated, or pre-determined documentation, and we will not issue documentation to support a claim we believe to be inaccurate or fraudulent.
You must be at least 18 years old and physically located in a U.S. state where a TravelMD-affiliated physician is licensed at the time of your consultation. States currently served: [LIST STATES]. You agree to provide accurate, current, and complete information.
Decisions about refunds, reimbursements, credits, waivers, or insurance claims are made solely by the relevant airline, hotel, insurer, or other third party under their own policies and at their sole discretion. We do not warrant that the Service will be uninterrupted, error-free, or that any particular result will be achieved.
TO THE FULLEST EXTENT PERMITTED BY LAW, TRAVELMD AND ITS AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, AND TRAVELMD'S TOTAL AGGREGATE LIABILITY ARISING OUT OF OR RELATING TO THE SERVICE SHALL NOT EXCEED THE AMOUNT YOU PAID TO TRAVELMD IN THE [TIME PERIOD] PRECEDING THE CLAIM. Payments are handled by Stripe in accordance with Stripe's own terms and privacy policy. Payment card data is transmitted directly to Stripe and is not stored on our servers.
